Physicians and Allied Health Providers
A CAQH application is required for specific allied health providers and physicians and must be completed, printed and mailed to HealthSpan for review.
The physician CAQH application may be obtained directly from CAQH at:

Ancillary Service Providers
Ancillary applicants may obtain a special HealthSpan application by calling 1-888-914-7726.

Review and Approval
HealthSpan will review your application against specific for participation in addition to the American Accreditation Commission / URAC standards. Submitting an application does not automatically make you a HealthSpan network provider. We will contact you regarding your participation status at the conclusion of the review process.
